Stanford Care Navigator is a messaging app that simplifies communication between caregivers, patients, relatives and providers (doctors, nurses, specialists). It is meant to be used primarily by patients, relatives, providers and pharmacists associated with Stanford Medical Hospital.
Stanford Care Navigator addresses communication gaps by creating a secure communication group for all parties connected with a patient. Caregivers, relatives, patients and providers can communicate these things in a secure chat group:
1. Patients relatives can be updated of the patients status (diagnosis, test results, surgery or chemotherapy schedules, next steps)
2. Caregivers and immediate family members can ask ZingHealths patient advocates, who are included in the secure chat group, questions concerning hospital logistics (where to show up, what to do, what to pay, when etc)
3. Any medical questions that ZingHealth patient advocates cannot field will be answered by the patients providers at the hospital or nursing facility.
4. Pictures of relevant test results, oral discharge summaries by nurses, oral consultations with nursing facility nurses etc can all be communicated via the care messenger.
The CareBoard within the app summarizes medications, appointments and ToDo lists for the patient. Medication details like the dosage, frequency, copays (if any), pickup locations etc are visible. You can also chat with Zing Health patient advocates on scheduling medication delivery. All pending appointments, the addresses, time and duration, copays if any and other details also are displayed in the CareBoard. Finally, checklists act as remindes for patients which can mark them as completed. Providers can track adherence via checklists.
